Tibial Tubercle Transferdistal Realignment Rehab Program
PHASE 1: 0-6 WEEKS
Heel Touch Weight Bearing (20%)
ROM:
progress from passive to active to resisted ROM as tolerated
Goal:
125° of flexion at 4 weeks post-op
Brace locked in full extension except for exercise/hygiene until 2 weeks post-op
May unlock brace at 2 weeks post-op
Strengthening: quad sets, co-contractions, isometric abduction/adduction.
PHASE 2: 6 WEEKS – 3 MONTHS
ROM:
progress to full ROM
Wean from brace
May advance weight bearing as tolerated
Strengthening: partial wall sits, SLR, etc.
Begin hamstring strengthening, theraband, open chain exercises at 10 weeks
PHASE 3: 3 MONTHS – 5 MONTHS
May begin treadmill, balance exercises, proprioception, sport-specific drills
Strengthening: Advance closed-chain exercises and single leg strengthening
May begin plyometrics and high-impact activities at 4 months post-op
Transition to home exercise program
