PHASE 1: 0-4 WEEKS
Sling at all times except for hygiene/exercises
ROM:
up to the ‘salute position’ only
Pendulum exercises and Codman’s only until 4 weeks post-op
PHASE 2: 4 WEEKS – 8 WEEKS
Wean from sling
ROM:
increase as tolerated, avoid stretching posterior capsule
Strengthening: active-assisted exercises, deltoid/rotator cuff isometrics
Avoid using exercise arcs that stress the posterior capsule
Keep all strengthening exercises below the horizontal plane
PHASE 3: 8 WEEKS – 3 MONTHS
ROM:
progress to full ROM w/o discomfort
Strengthening: may begin resistive exercises for RC and scapular stabilizers
Begin upper body ergometer
PHASE 4: 3 MONTHS – 5 MONTHS
Strengthening: advance to include ER and latissimus eccentric exercises
May begin muscle endurance activities (upper body ergometer)
Start aggressive eccentric strengthening and plyometrics at 4 months post-op
PHASE 5: 5 MONTHS+
Begin sport specific activities, increase joint velocity of motion
Transition to home exercise program
